If you are receiving Wajong (young disabled persons benefit) and you apply for conversion of your performance grant, you do not need to get in touch with your student counsellor or study advisor. In all other situations, the application for a performance grant provision should be made through your student counsellor or study advisor.

Conversion performance grant if you receive Wajong

1. Complete the ‘Aanvraag voorziening prestatiebeurs bij Wajong’ form.
2. Attach the supporting documents requested in the explanatory notes to your application.
3. Send the form to the address stated on it.

Other applications

1. Get in touch with your student counsellor or study advisor as soon as possible. In exceptional situations, the counsellor or advisor can apply for a performance grant provision on your behalf. The student counsellor or study advisor has a form for that purpose.
2. Complete question 1 (your details). The student counsellor or study advisor completes question 2 (application for provision). The application needs to be signed by both of you.
3. Attach the supporting documents requested in the explanatory notes to your application. Send the form to the address stated on it.
